The correlation between insulin resistance, leptin resistance and cortisol level in obese with and without diabetes mellitus type 2

This study aims to investigate the relationship between leptin and insulin resistance and cortisol level in obese with or without diabetes mellitus type 2 (DMT2). In this study, there were 75 participants between 20-45 years old, including 25 obese with DMT2 (BMI≥30kg/m2), 25 obese without DMT2 (BMI≥30kg/m2), and 25 normal-weight healthy individuals (BMI˂30kg/m2). After measurement BMI, 10ml of blood were drawn, 2ml were placed in EDTA tubes to immediately determination of HbA1C and 8ml were put in tube without EDTA to be centrifuged and serum be separated. Serum was used to determine the levels of cortisol, leptin, soluble leptin receptor, insulin and FBG. The results were showed that there is significantly higher IR in obese with DMT2 than Obese without DMT2 and control while leptin hormone was significantly higher and S-LR was significantly lower in obese with and without DMT2 comported to control. Moreover, cortisol level was slight significantly higher in obese with DMT2 and higher tendency in obese without DMT2 than control. The BMI has no significant correlations with LEP while it has high significant correlations with the both IR & S-LR. Interesting, both FBG and HbA1c have significant correlation with LEP and IR while only HbA1c has significant correlation with S-LR. However, Cortisol has significant correlations with BMI, FBG, and HbA1c, but not with LEP, IR and S-LR. As conclusion, both insulin and leptin control the secretion of cortisol in obese people and leptin resistance is one of the important causes of insulin resistance that result to DMT2.
